Get a Library Card

  1. Fill out a Library Card Application (choose one):
    1. Print and complete an application (must be signed in presence of Library staff).
      1. Library Card Application for adults, kids, and teens.
      2. Family Library Card Application for multiple people in the same household.
      3. Group Library Card Application for a business, school, or group home.
    2. Visit the Library to request an application.
  2. Complete the application process at the Library Checkout Desk.
  3. Provide photo identification and proof of current address:
    • An up-to-date government-issued I.D. works for both.
    • If you do not have an up-to-date I.D., please provide a bill or mail with your name and address on it.
  4. If you already have an account in our system, you may purchase a new physical library card for $1.
  5. A digital version of your library card is available in the CAFÉ Libraries mobile app.

get a library card in the appYou can get a temporary digital library card through the app.

  1. Download the CAFÉ Libraries App from the Google Play Store or Apple App Store.
  2. Select My Account from the home page.
  3. Scroll down and select Get a Library Card.
  4. Select the Oconomowoc Public Library from the home library drop-down menu.
  5. Enter your postal code. Select Continue.
  6. Select your appropriate city and county from the drop-down menu. Select Continue.
  7. Fill in your information accurately and create a temporary username and password. Click Submit.
  8. A temporary barcode, username, and password give you access to your online account.
  9. Visit the Library within a month to complete the registration and receive a permanent library card.
    1. Provide photo identification and proof of current address:
      • An up-to-date government-issued I.D. works for both.
      • If you do not have an up-to-date I.D., please provide a bill or mail with your name and address on it.

Library Card for Kids & Teens

There is no minimum age requirement to apply for a library card. Children under 18 must have a parent or guardian sign the library card application and provide the required photo identification and proof of address. All fines and fees are the responsibility of the parent or guardian.

Group Library Card

A Group Card enables a business, school, or group home to borrow library items.

To Obtain a Group Card
  1. Complete the Group Card Application.
  2. Provide the name and contact information for the person completing the application for the group and a list of staff members who are authorized to use the group card on company letterhead.
  3. The person completing the application must show a government-issued picture I.D. when applying.
